1. Eligibility:

• This giveaway is open to legal residents of the state of Iowa, United States, who are aged 65 or older at the time of entry.

2. Giveaway Period:

• The giveaway starts on April 15, 2026 at 12:00 a.m. and ends on May 15, 2026 at 11:59 p.m. (the "Giveaway Period").
• Entries submitted before or after the Giveaway Period will not be eligible.

3. How to Enter:

• To enter, participants must complete the mailed survey and return in the included business reply envelope. One giveaway entry will be earned for each qualifying returned survey.
• Each participant is allowed one entry per person.

4. Winner Selection:

• One winner will be randomly selected from all eligible participants following the Giveaway Period.

   

5. Prize:

• The randomly selected winner will receive a gift certificate to Hugo's Woodfired Grill, valued at $100.
• The prize is non-transferable, non-refundable, and cannot be exchanged for cash or other alternatives.

6. Winner Notification:

• The selected winner will be notified via email within 5 days after the Giveaway Period has ended.

7. Claiming the Prize:

• The selected winner must respond via email within 5 days of receiving the winner notification email, confirming receipt and accepting the prize and its conditions.
• If the initially selected winner does not respond within the time allotted, a new winner will be selected and the notification and claiming process will repeat with the newly selected winner until the prize is claimed.
• Once the prize is claimed, the winner is required to retrieve their gift card from Hugo's Woodfired Grill.

• Upon securing their prize, the winner assumes all responsibility. WesleyLife is not responsible for lost or stolen gift cards once the prize has been distributed.

8. Publicity:

• By entering the giveaway, participants grant WesleyLife the right to contact them with marketing information via email and mail. 

9. General Terms:

• WesleyLife reserves the right to disqualify any entry that violates the rules, is incomplete, or contains false or misleading information. 
• The Giveaway is subject to all applicable federal, state, and local laws and regulations. 
• If the Giveaway is interrupted or cannot be conducted due to technical errors, fraud, or other causes beyond the control of WesleyLife, the company reserves the right to cancel or modify the Giveaway. 

10. Privacy:

•  Information collected from participants will be used for the purpose of this giveaway and future marketing communications in accordance with the WesleyLife privacy policy.
