2019 WellAhead Program

A few simple reminders can determine a lot about your health and well-being. When you are armed with knowledge, you can take a more proactive role in your health. See below for ways you can gain that knowledge, become involved with the WesleyLife WellAhead program, and earn rewards!

To earn the discounted wellness rate for 2020 medical insurance premiums, team members will need to complete the following 3 steps:

Step 1: Know Your Numbers

Free Health Screening events were held March 5 - April 16, 2019, to enable team members to complete these three steps:

  1. Biometric Screening (or complete a physician form, see below)
  2. Health Risk Assessment & Resilience Scale
  3. Nicotine-Free Pledge**

**If you are a tobacco user, you can still earn the wellness rewards by completing the HealthyGuidance Tobacco Cessation program through ComPsych by September 1 2019. Ready to begin? Call anytime at (800) 272-7255 to request an appointment with a Tobacco Cessation Specialist.

Biometric Screening:

If you missed your free Health Screening, it's not too late to earn WellAhead benefits! Complete your Physician Form (details below) to fulfill the requirements of Step 1. 

Physician Forms:

Simply take this form to your personal physician, then send the completed form to Jennifer Umphfleet at UnityPoint Health by 9/1/2019. 

NOTE: Team members who choose to complete the physician form will be responsible for completing their Health Risk Assessment, Resilience Scale, and Nicotine Free Pledge on their own time. Physicals between 9/1/2018 and 9/1/2019 will be accepted.

Health Risk Assessment & Resilience Scale:

After you have completed the onsite biometric screening or the physician form, you'll be ready to take the online assessments! 


What do I need to take the assessments?

  • Use a WesleyLife computer (the survey does not work on tablets or non-WesleyLife computers).
  • Sign into the computer as you. This username and password will be the same as your WesleyLife email login information. Not sure what your login is? Contact the Help Desk at (515) 271-6876.
  • You'll need your numbers from your onsite biometric screening or physician form. Don't have your numbers? Contact

Nicotine-Free Pledge:

Download and complete the Nicotine-Free Pledge. Scan the completed form to by September 1, 2019. If you are a tobacco user and are choosing to complete the reasonable alternative cessation program, please follow instructions listed under step 1.

Well-Being ChampionStep 2: Become a Champion

Participate in quarterly Core4 Well-Being Champion challenges in 2019, earn points, and receive additional incentives. Sign-ups and details about each quarterly challenge will be sent via email.

  • Q1 - Be Connected: Disconnect to Connect | February 25- March 8
  • Q2 - Move Naturally: Trek Across the U.S. | June 3 - 14
  • Q3 - Right Outlook: Discover Deeper Meaning | September 16 - 27
  • Q4 - Eat Wisely: Gut Check | November 11 - 22

The challenges, designed around the quarterly Core4 focus, offer team members the opportunity to participate in a variety of activities and aspects of well-being. All challenges have specific guidelines for how team members will be able to earn their 50 points. 

Information about each of these challenges will be sent via the Weekly Core4 Newsletter and will be posted in each of our Communities for Healthy Living, as well as our WesleyLife at Home campuses. Your wellness team will have sign-ups available the week prior to each challenge.

Step 3: Reap the Rewards

If you complete Step 1: Team members who elect WesleyLife medical coverage can save up to $1,600** a year on their premiums when they complete Step 1 (the three Know Your Numbers requirements explained above), qualifying them for the wellness premium rate.

**$1,600 is the estimated cost savings for a family medical plan.**

DISCLAIMER: Team members on the Bronze medical plan are not eligible for the wellness premium discount.

If you complete Step 1 & Step 2: After completing Step 1, team members are eligible to earn Well-Being Champion rewards. Each quarterly challenge = 50 points + badge

About the Health Risk Assessment & Resilience Scale

Health Risk Assessment: Use your health assessment personal report to determine health changes you'd like to make in 2019. Share the report with your physician to assist with comprehensive management of your health care. This assessment will take you approximately 20-30 minutes to complete.

Resilience Scale: Take the resiliency scale to learn about the five core areas of resilience. When finished, you'll receive a personalized report that will give you tips and strategies to help you develop your own coping plan. This assessment will take you approximately 1-2 minutes to complete.
