Hospice Care
When individuals are diagnosed with a life limiting illness, they enter a strange, frightening world of alarming statistics, unfamiliar medical terminology and difficult decisions. Often times patients feel overwhelmed, are unsure where to turn for care, and are concerned about becoming a burden to their family and loved ones.
WesleyLife's Hospice team help sort through available options, offer knowledgeable recommendations, and provide the education needed for patients and family to make informed personal care decisions based on the patient's goals, values and wishes. WesleyLife Hospice does not focus solely on the physical needs of the patient; instead the Hospice team coaches a patient and their family through the emotional and spiritual aspects of the illness.
What is Hospice?
Hospice is a comprehensive, interdisciplinary program designed to provide hope, care and comfort to individuals with a life-limiting illness wherever they call home.
WesleyLife Hospice Criteria:
- Life expectancy of six months or less
- Referral from personal physician
Insurance Requirements:
- WesleyLife Hospice will not turn anyone away because of inability to pay for services
- Medicare, Medicaid, and most private insurance plans cover hospice benefits
Entering WesleyLife Hospice:
The WesleyLife Hospice team will meet with the individual and their family to develop a flexible, individualized plan that covers the patient's wishes. The multi-disciplinary team provides the following services:
- Medical Director directs physical care
- Nurses combine clinical skills with caring hearts
- Patient Life Advocate links to other resources and oversees process
- Spiritual Care Coordinator provides guidance and support
- Home Health Aides assist with personal care
- Massage/healing touch therapist helps reduce anxiety
- Bereavement Counselor offers support and counseling
- Volunteers provide caregiver relief, companionship and help
- Music and pet therapies bring enrichment and reduce anxiety
